Maternity Exemption Certificate - replacement?

Hey all - I don't suppose anyone knows how I go about getting a replacement card for my maternity exepmtion - #I cannot find it for love nor money and I know you don't have to have it to pick up a perscription my dentist requires to produce it and I have looked high and low (baby brain!) and it's no doubt somewhere safe alongside my �20 next giftcard I also can't find!!! I've looked ont he net for a phone number or website but can't find anything and was wondering if there's anything on anybodys cards to say if found return to etc?
